I'm having to use a different computer this weekend and I've just tried logging in to make sure it works. However I'm getting the problem:
"The save file you have selected is protected to ensure that the assets in your mod folder have not been changed."

I installed it from fresh:
 Install civ IV
 Install Civ BTS
 Install BTS Patch 3.17
 Copy 40Civs mod into the MODS folder

The game loads fine and so does the mod. The "About this build" reports it as:
Civ Version: 317
Save Version: 301
Build Version: 3.1.3.1 (10417)
Build Date: Mon May 19 14:20:10 2008
Build: FINAL RELEASE
Mod Name: mods\40Civs\

I've found various threads on this suggesting it's a FAT32 problem, but the machine I'm using is on NTFS. I've tried removing the customassets folder and that doesn't work either.

I'm out most of tomorrow, but if anyone can spot something obvious I've done wrong it would be most appreciated!

Stupid me - I've just realised my mistake. I'll leave the answer here in case I (or someone else) does it again!

The 40Civs directory has to be in the MODS folder within the Beyond The Sword directory. I had it in the Civilization 4\MODS directory

Sorry - I'll blame it on being tired.

No.  Blame it on the absurd propogation of games that distribute their files into all kinds of different folders and subfolders in general.  Blame it on Civ giving you two different directories that look the same and work the same 90% of the time, in particular.
